On November 8, 2005, voters approved a 1% income
tax to support Logan Elm Schools. This tax is administered by the Ohio
Department of Taxation. If you have a question regarding payment of the 1%
income tax, please call 1-800-282-1780. Listen to the automated message
and select the option for “School District Income Tax Information”.

Go to their home page at www.tax.ohio.gov

Click on “Get a tax form” under “Popular services

There will be a statement at the top that says: “Click here to search all tax forms and related instructions.” Click “here”.

This will take you to a search engine. Use the “Tax Type” dropdown menu to select “School District Income Tax”. Click “Search”.

This will bring up a list of all school district income tax forms. It will include forms for the current and previous tax years. Be sure to select the right form and year based on your need.
The most commonly requested forms are:
School District Estimated Income Tax Payment
Voucher (SD 100ES) – If your employer is
not withholding the school district income tax from your check, you can
make quarterly estimated payments using these forms to avoid a penalty
at the end of the year.
